Bluestone Vista at Mankato Clinic is an innovative and unique approach to delivering healthcare by providing mobile primary care services to patients living in assisted living and memory care communities in and around the Mankato area.

Join our team as a Nurse Practitioner or Physician Assistant where our care model delivers primary care that is preventative, proactive and includes all members of the care team, patients and their families.

Position Includes:

\xc2\xb7 Provide on-site primary care to patients within assisted living and memory care communities.

\xc2\xb7 Full-time providers care for patients Monday-Thursday with Fridays being an administrative day.

\xc2\xb7 Manage a panel of approximately 150-200 patients (approximately 12-16 patients/day)

\xc2\xb7 Develop and implement clinical plans of care & discuss care plans with facility nurse

\xc2\xb7 Meet best practice and quality measure requirements

\xc2\xb7 Rotate covering our online portal (1 weeknight and approximately 6-7 weekends/year)
Job Requirements:

• Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
• Must have and maintain a valid Minnesota Practice License
• Board Certified AANP, ANCC, or NCCPA
• Ability to obtain and maintain DEA certification for the State of Minnesota
• Minimum of 2 years professional clinical experience in Family Practice or Internal Medicine. Geriatric training or interest preferred.
• Ability to effectively communicate with others regardless of ethnic, religious, or economic status; strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver\xe2\x80\x99s license and minimum state required liability auto insurance; local travel is required
• Three professional references, drug screen, health assessment, background check.
