Job Description

:

• 16 Hours a week / 32 hours a pay period

• Occasional On call, and occasional weekends

Summary:

The Assisted Living Clinical Manager is responsible for the overall coordination of a client\'s health care needs. They initiate the physician orders upon admission and develops a treatment plan that specifically meets the individuals\' needs. They communicate with the physician, family and health care team to assure that all physical, psychosocial and emotional needs are met.

Duties:

Assesses, develops and implements comprehensive care plan for each tenant. Obtains and transcribes Physician Orders. Performs nursing care duties for tenants. Practices proper infection control to ensure safe environment for tenants and staff. Staffs, supervises, manages and trains LPN\'s and NAR\'s. Completes charting. Gathers and documents data on admissions and discharges. Initiates, reports and documents nursing orders. Communicates daily tenant care needs and/or significant changes in condition with the health care team.

Qualifications:

Must have current registration, as a Registered Nurse or LPN, in the State of Minnesota. Must have successfully completed an approved School of Nursing. Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider. Must possess the ability to communicate verbally and in writing when directing the care of residents. Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment, after criminal background study, per guidelines of Minnesota Department of Human Services.
