:

The Associate Community Health Worker is a trusted community member who acts as a liaison between a community and health/social service systems. The Associate Community Health Worker collaborates with other team members to support neighbors in achieving their goals health and social goals. They will advocate for neighbors from the community they serve to ensure they receive the services and resources they may need with the assistance of more senior team members. The Associate CHW will partner with team members to serve with clinical and community partners. May participate in neighbor outreach and data collection as well as utilize shared lived experience to further trusting relationships with communities.

Preferred Qualifications:

• 1 year Experience working in community services, community health, or a similar environment

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
