:We are seeking a Behavioral Health Case Manager.As an Addiction Case Manager at our Doorway-Berlin, you will play a pivotal role in coordinating and overseeing the care and support services for individuals struggling with addiction. Your primary responsibility will be to assess clients\' needs, develop comprehensive treatment plans, and ensure they receive appropriate services and resources. You will collaborate closely with healthcare professionals, social workers, and community organizations to provide holistic care that addresses both the physical and psychological aspects of addiction.Your duties will include conducting intake assessments, evaluating clients\' progress, and advocating for their needs within the healthcare system. You will also provide counseling and support to clients and their families, helping them navigate the challenges of addiction recovery. Documentation and record-keeping are essential aspects of the role to ensure accurate tracking of clients\' treatment plans and outcomes.The ideal candidate will have a background in social work, psychology, or a related field, with experience in addiction counseling or case management.
