As our valued partner, we at Phamily are assisting in the hiring of CHRONIC CARE NAVIGATOR / CARE MANAGER for our partners at Insight Medical in their Dearborn, Michigan location. The candidate for this role must reside in the state of Michigan. This role is primarily remote, although the ideal candidate will be within traveling distance for occasional visits to the office for trainings or other.

The Chronic Care Navigator implements, manages, and coordinates all Chronic Care Management Program activities. You will directly engage, manage, and triage complex clinical needs.

YOU WILL BE RESPONSIBLE FOR:

Developing a proper understanding of chronic pain and family medical care requirements for optimal, coordinated population health

Proactive patient management via text using Phamily platform

Proactive patient management via phone calls

Triage and assessment of patient health status

Coordination of care between patient and provider

Active review of patient charts to assess health needs and gaps

Providing a positive patient experience for patients and families through courteous digital interactions

Routing patient escalations accurately and expeditiously

Making referrals to appropriate staff when necessary

YOU MUST FORMALLY RESIDE IN:

Michigan

JOB REQUIREMENTS

• High school diploma, GED, or suitable equivalent
• Must be able to travel periodically for in office trainings to Dearborn Michigan.
• Must be able to achieve CPR certification within 30 days of employment.
• Vaccinated for COVID-19 unless unable to do so due to medical or sincerely held religious beliefs.

THE IDEAL APPLICANT FOR US:

• Is a Certified Medical Assistant or Licensed Practical Nurse
• Demonstrates excellent skills in verbal and written English communications for safe and effective patient care, in order to meet documentation standards.
• Previous experience and familiarity with EMRs.

SALARY: $17.00 - $19.00 per hour

POSITION HOURS: 9am-5pm (primarily)
