A medical group has open positions for Care Coordinators to perform a variety of care management services for qualifying patients. After the mandatory 2 week training is completed, work hours are Monday through Friday from 8:30 a.m. to 5 p.m.
Responsibilities:
Communicate with patients to follow-up on care coordination, support, and manage compliance with the care management programs
Document all client communications accurately
Communicate regarding patient emergent needs and/or life-threatening episodes to ensure care plans are complete and accurate
Inform supervisor of all issues pertinent to the care plan process or any other issues
Use care management platform, secure email, softphone/VOIP, Electronic Records, etc.
Adhere to documentation protocols and best practices for daily work logs, client issues, and internal communications
Perform other duties as assigned
Job Requirements
Qualifications:
Must have Medical Assistant experience in a Primary Care Practice
Must be Bilingual Spanish/other language is desired but not mandatory
Must have knowledge of medical terminology
Must have critical thinking ability and be able to discern next set of questions or next course of action
Must have the ability to develop and build trust relationships with patients
Must have excellent MS Office skills and experience working with Electronic Medical Records Software
Must be motivated with strong communication, organizational, multi-tasking, and time management skills
Must be able to follow all protocols, practices, rules, regulations, as well as, having the ability to maintain confidentiality and exercise discretion in dealing with all matters
