We are looking for a Care Coordination & Utilization Management Supervisor to assume responsibility for coordinating and supervising assigned department(s) in the and Health Ministries. This is inclusive of financial responsibility, departmental performance, performance management, and quality improvement programs.
In this role, you will identify all customer groups, assess their needs, and coordinate the department to successfully achieve service line objectives. You\xe2\x80\x99ll ensure department policies and procedures are followed and bring suggestions for change to leadership, while assisting with multi-disciplinary business planning and program growth development.
About This Position
This is a full-time, salaried position supporting both our Ontario and Baker City Hospitals. If selected you will be expected to have an onsite presence in both locations and must be willing to travel accordingly. Mileage will be reimbursed according to organizational policy and current federal reimbursement guidelines.
GENERAL REQUIREMENTS
Education/Experience:
Must possess knowledge of healthcare operations as normally obtained through a Bachelor\xe2\x80\x99s degree in Nursing or a related field with 4 years of related nursing experience or an equivalent combination of education and experience.
Master\'s Degree in healthcare or business administration preferred.
Two years of supervisory experience preferred.
Experience with Critical Access Hospitals and Swing Bed Programs desired.
Licensure/Certification:
Must have active Oregon Nursing (RN) license, issued prior to hire date.
Must provide licensure or proof of application for Idaho Nursing (RN) License within six months of hire
ESSENTIAL FUNCTIONS:

Successfully executes the established plans and services of the functional areas of responsibility through analysis, measurement and monitoring, reporting, and adjusting the direction, allocation and utilization of available resources.
Revenue Management:
Understands the integration between clinical activity and revenue management.
Ensures the accuracy of documenting services and supplies provided to the patients.
Ensures compliance with the Method 2 Billing requirements for the Critical Access Hospital
Assists in the preparation of annual department budget and monitors departmental performance to the budget and adjusts expenditures accordingly.
Ensures accuracy of and approves payroll for direct reports.
Responsible for the hiring, training, retention and supervision of staff located at Ontario and Baker City sites. Participates in the development and education of staff, including fostering teamwork, providing performance feedback, mentoring and coaching associates, and scheduling. Collaborates with CRM Director on competency assessments, performance evaluations, counseling and/or conflict resolution.
Develops assigned aspects of the organizational strategic and tactical plans in concert with established planning processes.
Utilizes change management tools in identifying opportunities for patient care improvement.
Assures compliance with all applicable SAHS and RHM policies and procedures, Trinity Health Case Management and Utilization Review Excellence Standards and maintains, CMS, Joint Commission and other specific regulatory standards.
Identifies best practices and implements innovative approaches to improve care.
Participates on teams regarding care coordination efforts as appropriate.
Establishes and maintains a positive and productive working relationships with all relevant customers and suppliers.
Facilitates and manages team performance to the established performance levels by effective use of performance improvement, human resources, and financial skills, techniques and communications.
Manages/oversees UR committee functions.
Oversees the Swing Bed Program in Baker City in conjunction with the Clinical Resource Managers and other nursing leadership.
Serves as liaison to Senior Management and Medical Staff for the functional areas of responsibility.
Ensures compliance with all applicable SARMC policies and procedures.
Serves in a staff role based on the department staffing plans and day-to-day requirements in Ontario and Baker City. Duties generally include, but are not limited to, care coordination, utilization management, discharge planning, and integration of social services into patient care as appropriate.
Must be willing to travel and work onsite at both Ontario and Baker City locations.
