Case Manager/Social Worker

Tempe Post-Acute is a 74 bed skilled nursing facility in Tempe, Arizona. Our address is 6100 S. Rural Rd, Tempe, Arizona, 85283.

:

Working closely with the clinical team creating treatment plans, coordinating care, gauging progress, and discharge plans. Oftentimes case managers work with patients, physicians, social workers, and families.
