Summary of Responsibilities:



The Case Manager is responsible for the provision of home-based services, case management, as well as direct intervention services to youth and their families and/or therapeutic treatment parents across the state of Alaska. Duties include but are not limited to:

  • Crisis intervention
  • Advocacy
  • Assessment
  • Referrals and discharge planning
  • Treatment planning
  • Individual, family, and group skill development services
  • Supporting youth access to services/ transportation
  • Interdisciplinary teamwork
  • Community liaison
  • Participation in a rotating on-call service completing these duties as outlined in policy for when on-call
  • Abides by all safety rules and regulations of the agency
  • Carries out all duties according to AK Child & Family policies, the AK Child & Family mission, and the AK Child & Family philosophy of care
  • Under the supervision of the Clinical Case Management Supervisor, case managers are responsible for implementing appropriate treatment strategies and for timely and satisfactory documentation.
  • Assumes additional duties as assigned
  • The Case Manager adheres to standards and regulations as set forth by the agency, the Joint Commission, OCS licensing, DBH and Medicaid.

Pay:
  • $19.00/hour +depending on education and experience
  • 3% pay increase after successful completion of 6-month Intro Period.





Supervision and Coordination of Services:

  • The Clinical Case Management Supervisor supervises Case Managers.
  • The Case Manager coordinates services with Behavioral Health Specialists, Therapeutic Treatment Parents, and members of the Treatment Team as appropriate.





Qualifications:



Knowledge, Skills and Abilities:
  • The Case Manager must have the ability to relate to and experience in working with emotionally disturbed children/adolescents and their families.
  • Demonstrated ability to complete assessments, treatment planning, parent training, advocacy skills and case management skills are also required.
  • The ability to provide written and oral communication in English, along with demonstrated interpersonal/social skills is essential.
  • Creativity/flexibility must be demonstrated in the delivery of services.
  • Possesses a demonstrated commitment to valuing diversity and contributing to an inclusive working and learning environment.
  • Excellent time management, organizational skills, and computer skills (MS Office Suite) are also necessary qualifications.
  • The CM must demonstrate the ability to function as a member of the interdisciplinary treatment team and display an understanding and sensitivity to serving a culturally diverse and/or special needs population.
  • The CM must possess a valid driver\xe2\x80\x99s license and ability to travel within the community.

Education and experience:
  • Must hold a bachelor\xe2\x80\x99s degree in psychology, counseling, or social work, or a closely related field as identified by inclusion of significant coursework in psychology, child development and related studies and have one year experience working with emotionally disturbed children/adolescents, preferably including case management skills (preferred); or
  • Five years\xe2\x80\x99 experience working with emotionally disturbed children/adolescents.
  • Knowledge in psycho educational skill building, cognitive and reality-based therapies, sexual abuse and neglect, and family skill building is preferred.





Physical Demands:

  • Must be able to access AK Child & Family and community buildings for meetings and agency activities.
  • Is required to have a valid driver\xe2\x80\x99s license and an adequately insured personal vehicle for reliably getting to and from work responsibilities as work responsibilities require.
  • He/she will transport self and others as needed to meet with schools, families and other involved parties.
  • He/she must produce in a timely manner, assessments and other documentation required by AK Child & Family, insurance and referral sources.
  • Must be able to navigate internet-based systems, electronic health record, and must be able to use a copy machine, fax machine, as well as to produce thorough youth records and prepare reports using a computer.
