Job Title: Case Manager



We are seeking a dedicated and compassionate Case Manager to join our Malama Kupuna Care team. This full-time position focuses on providing exceptional support and client care. As a Case Manager, you will play a crucial role in assisting our clients with their needs and ensuring they receive the highest level of care and service.




Responsibilities:

Conduct comprehensive assessments of clients' needs and develop personalized care plans. Coordinate and facilitate access to necessary services and resources for clients. Maintain regular communication with clients to monitor progress and address any concerns or changes in their care needs. Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless delivery of services. Document and maintain accurate records of client interactions and care plans. Provide ongoing support and guidance to clients and their families, ensuring they understand available options and resources. Advocate for clients' needs and rights, ensuring they receive appropriate care and services. Participate in team meetings and contribute to the continuous improvement of care processes.


Requirements:

Proven experience in case management, client care, or a related field. Strong interpersonal and communication skills, with the ability to build rapport with clients and their families. Exceptional organizational skills and attention to detail. Ability to work independently and manage multiple cases simultaneously. Proficient in using computer systems and software for documentation and communication. Language proficiency as required for client communication.
