As our organization continues to expand, we are seeking compassionate Case Managers to join our vibrant team.Case Managers at Mental Health Kokua play a crucial role in providing recovery-oriented services to our consumers. They support individuals with a spectrum of psychiatric disabilities, effectively managing stressful situations and emotional conflicts.Key Case Manager Responsibilities:

  • Assessment: Conduct prompt behavioral health assessments and facilitate referrals for psychiatric and medication evaluations.
  • Service Planning: Develop and revise personalized treatment plans, coordinate services and referrals, and provide ongoing support to monitor progress.
  • Follow-Up: Ensure continuity of care post-hospitalization through necessary outreach services.
  • Hospitalization Support: Assist consumers in preventing re-hospitalization for psychiatric care.
  • Coordination: Verify insurance coverage, collaborate with healthcare providers, and coordinate efforts with fellow Case Managers.
  • Adaptability: Embrace evolving responsibilities in line with organizational and industry standards.
  • Recovery Model: Implement and support consumers in achieving health and wellness goals through the recovery model.
  • Documentation: Maintain accurate and timely clinical records, including progress notes and required forms.
  • Clinical Supervision: Participate in clinical supervision sessions to discuss consumer progress, ethical considerations, and cultural competency.
  • Program Compliance: Adhere to Mental Health Kokua\'s case management program guidelines and ethical standards.
  • Additional Responsibilities: Perform other duties as assigned by management.
Case Manager Requirements:
  • Bachelor\'s degree in psychology, sociology, social work, or related human/social services field preferred.
  • Proficient in computer skills with the ability to document consumer records promptly.
  • One and a half (1 \xc2\xbd) years of specialized training in case management or human services preferred.
  • Competency in providing services for dually diagnosed individuals is advantageous.
  • Successful completion of a criminal background check and pre-employment drug screening.
  • Possession of a valid Hawaii driver\'s license, vehicle insurance, and safety inspection.
  • Clean driving record verified by a Traffic Abstract.
  • Current TB Clearance or TB X-Ray.
  • Current CPR & First Aid Certificate.
  • Strong interpersonal, problem-solving, and communication skills.
  • Ability to work collaboratively in a team and adapt flexibly to various situations.
