Position:

Centralized Admissions Coordinator

Location:

Primary location in Avon, CT. Must be willing to travel as needed.

Type:

Full-Time Monday-Friday Per Diem weekends




Are you passionate about ensuring seamless transitions for residents into long-term care facilities? Join our team as a Centralized Admissions Coordinator, where you'll play a pivotal role in guiding individuals and their families through the admission process with care and compassion.

Responsibilities:

Plan, organize, and execute the admission process, ensuring compliance with all regulations and guidelines. Process admissions applications and admit new residents efficiently and effectively. Collaborate with nursing, MDS, and rehab departments to facilitate discharge plans for residents. Provide guidance and support to existing facility Admissions Coordinators, offering coverage when necessary. Offer timely referrals to appropriate agencies for resident/family assistance, such as conservatorship and Medicaid applications.

Qualifications:

Previous experience in a long-term care facility within Admissions, Social Work, or Nursing required. LPN license preferred.*
Knowledgeable of admissions and discharge procedures, current laws, residents' rights, and documentation requirements. Strong attention to detail, follow-through skills, and ability to prioritize tasks. Innovative mindset to find creation solutions for resident needs while maintaining a customer focus. Possess a vehicle and valid drivers license Willing to travel to different facility locations; primary work location at Corporate office in Avon, CT Ability to remain calm in stressful situations and communicate professionally with residents, staff, and family members.
