Reports to: Director of Staff Development (DSD) Department: Nursing

Purpose of Your Job Position:

The primary purpose of your job position is to provide each of your assigned residents with routine daily activities and nursing care and services in accordance with the resident\xe2\x80\x99s assessment and care plan, and as may be directed by your supervisors.

Delegation of Authority:
As a Certified Nursing Assistant, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Duties and Responsibilities:
Administrative Functions:
\xe2\x97\x8fUse the wristband or photo to identify residents before administering treatments, serving meals, etc., as necessary.
\xe2\x97\x8fRecord all entries on flow sheets (ADLs. I&Os, percentage of meals eaten, vitals, bowel & bladder) in an
informative, descriptive and legible manner. Record additional entries as requested/required by the charge nurse.
\xe2\x97\x8fUse only authorized abbreviations established by this facility when recording information.
\xe2\x97\x8fReport all changes in the resident\xe2\x80\x99s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
\xe2\x97\x8fReport all accidents and incidents observed as they occur.
\xe2\x97\x8fTransports residents to assigned locations for activity.
\xe2\x97\x8fMonitor and supervise residents at all times to ensure resident safety.
\xe2\x97\x8fAssist in passing meal trays to residents at meal times. Ensure the correct tray is given to the correct resident.
\xe2\x97\x8fAssist and participate in a hydration program.
\xe2\x97\x8fBe knowledgeable and follow all safety procedures and regulations established in the employee handbook and other policies.
\xe2\x97\x8fAssist patients at meal times

Admission, Transfer, and Discharge Functions:
\xe2\x97\x8fEnsure that the resident\xe2\x80\x99s room is ready for receiving the resident (i.e., bed made, name tags up, etc.).
\xe2\x97\x8fGreet residents and escort them to their room.
\xe2\x97\x8fIntroduce residents to his/her roommate, if any, and other residents and personnel as appropriate.
\xe2\x97\x8fMake residents comfortable (i.e., put to bed, get water, etc.).
\xe2\x97\x8fShow the resident the bathroom and whether it is shared or not.
\xe2\x97\x8fOrient the resident to the call light system.
\xe2\x97\x8fInventory and mark the resident\xe2\x80\x99s personal possessions. Hang or fold and store as needed.
\xe2\x97\x8fExplain facility laundry procedures.
\xe2\x97\x8fStore resident\xe2\x80\x99s clothing.

\xe2\x97\x8fAssist residents with packing their personal possessions when they are being transferred to a new room, or when
being discharged.
\xe2\x97\x8fTransport residents to new rooms or to the receiving area.
\xe2\x97\x8fAssist with loading/unloading residents from vehicles as necessary.

Personnel Function:
\xe2\x97\x8fPerform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.
\xe2\x97\x8fFollow work assignments, and/or work schedules in completing and performing your assigned tasks.
\xe2\x97\x8fCreate and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
\xe2\x97\x8fReport all complaints and grievances made by the resident.
\xe2\x97\x8fNotify the facility when you will be late or absent from work. Inform the facility as soon as you are aware that you will not be able to report for duty. It is policy to give us at least 2 hours notice before the beginning of the shift to find your replacement.
\xe2\x97\x8fNotify your supervisor when leaving for a break or meal period and inform her/him when you return. Ensure that someone will be responsible for your assignments while you are gone.
\xe2\x97\x8fReport occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to your supervisor. Complete an incident report.

Staff Development:
\xe2\x97\x8fAttend and participate in scheduled training and educational classes to maintain current certification as a Nursing Assistant.
\xe2\x97\x8fAttend and participate in scheduled orientation programs and activities.
\xe2\x97\x8fAttend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management, and bloodborne pathogens standard.
\xe2\x97\x8fAttend and participate in advance directive in-service training programs as scheduled.

Safety and Sanitation:
\xe2\x97\x8fParticipate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.
\xe2\x97\x8f Wash hands before and after performing any service for the resident.
\xe2\x97\x8fKeep the nurses\xe2\x80\x99 call system within easy reach of the resident.
\xe2\x97\x8fImmediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility
\xe2\x97\x8fFollow established safety precautions in the performance of all duties.
\xe2\x97\x8fKeep residents\xe2\x80\x99 personal possessions off the floor and properly stored.
\xe2\x97\x8fKeep floors dry. Report spills immediately.
\xe2\x97\x8fKeep excess supplies and equipment off the floor. Store in designated areas.
\xe2\x97\x8fEnsure that residents\xe2\x80\x99 personal items are stored safely and securely for their protection and for the protection of others.
\xe2\x97\x8fBefore leaving the work area for breaks, or at the end of the work day, store all tools, equipment, and supplies.
\xe2\x97\x8fReport all hazardous conditions and equipment to the Nurse Supervisor/Charge Nurse immediately.
\xe2\x97\x8fReport all safety violations.
\xe2\x97\x8fFollow established smoking regulations. Report all violations
\xe2\x97\x8fReport any communicable or infectious disease to the Director of Nursing Services and/or to the Infection Control Coordinator.
\xe2\x97\x8fFollow established isolation precautions and procedures.
\xe2\x97\x8f Wash hands before entering and after leaving an isolation room/area.

\xe2\x97\x8fFollow established procedures in the use and disposal of personal protective equipment.
\xe2\x97\x8f Wear and/or use safety equipment and supplies (e.g., back brace, mechanical lifts, etc.) when lifting or moving residents.
\xe2\x97\x8fReport missing/illegible labels and MSDSs to your supervisor.

Equipment and Supply Function:
\xe2\x97\x8fUse only the equipment you have been trained to use.
\xe2\x97\x8fOperate all equipment in a safe manner.
\xe2\x97\x8fUse only the equipment and supplies necessary to do the job. Do not be wasteful. Do not hoard linens or other supplies.
\xe2\x97\x8fReport defective equipment to the Nurse Supervisor/Charge Nurse.
\xe2\x97\x8fInform the Nurse Supervisor/Charge Nurse of your equipment and supply needs.

Care Plan Functions:
\xe2\x97\x8fReview care plans daily to determine if changes in the resident\xe2\x80\x99s daily care routine have been made on the care plan.
\xe2\x97\x8fInform the Nurse Supervisor/Charge Nurse of any changes in the resident\xe2\x80\x99s condition so that appropriate information can be entered on the resident\xe2\x80\x99s care plan.
\xe2\x97\x8fEnsure that your notes reflect that the care plan is being followed.

Resident Right:
\xe2\x97\x8fMaintain the confidentiality of all resident care information
\xe2\x97\x8fEnsure that you treat all residents fairly, and with kindness, dignity, and respect.
\xe2\x97\x8fEnsure that all nursing care is provided in privacy.
\xe2\x97\x8fKnock before entering the resident\xe2\x80\x99s room
\xe2\x97\x8fReport all grievances and complaints made by the resident to the Nurse Supervisor/Charge Nurse.
\xe2\x97\x8fReport all allegations of resident abuse and/or misappropriation of resident property.
\xe2\x97\x8fHonor the resident\xe2\x80\x99s refusal of treatment request. Report such requests to your supervisor.

Working Conditions:
\xe2\x97\x8f Works throughout the nursing service area (i.e. drug rooms, nurses\xe2\x80\x99 stations, resident rooms, etc.).
\xe2\x97\x8fMoves intermittently during working hours.
\xe2\x97\x8fIs subject to frequent interruptions.
\xe2\x97\x8fIs involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
\xe2\x97\x8fIs subject to hostile and emotionally upset residents, family members, personnel, and visitors.
\xe2\x97\x8fCommunicates with nursing personnel and other department personnel.
\xe2\x97\x8f Works beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
\xe2\x97\x8fIs subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
\xe2\x97\x8fAttends and participates in continuing educational programs.
\xe2\x97\x8fIs subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
\xe2\x97\x8fIs subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
\xe2\x97\x8fMay be subject to the handling of and exposure to hazardous chemicals

Experience:

  • Must be a licensed Certified Nursing Assistant in accordance with laws of this state
Specific Requirements:
\xe2\x97\x8fMust be able to read, write, speak, and understand the English language. Others must be able to understand your speech.
\xe2\x97\x8fMust possess the ability to make independent decisions when circumstances warrant such action.
\xe2\x97\x8fMust possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
\xe2\x97\x8fMust possess the ability and willingness to work harmoniously with other personnel.
\xe2\x97\x8fMust have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
\xe2\x97\x8fMust be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
\xe2\x97\x8fMust be able to relate information concerning a resident\xe2\x80\x99s condition.
\xe2\x97\x8fMust not pose a direct threat to the health and safety of other individuals in the workplace.
Physical and Sensory Requirements: (With or Without the Aid of Mechanical Devices)
\xe2\x97\x8fMust be able to move intermittently throughout the work day.
\xe2\x97\x8fMust be able to cope with the mental and emotional stress of the position.
\xe2\x97\x8fMust be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
\xe2\x97\x8fMust function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
\xe2\x97\x8fMust meet the general health requirements set forth by the policies of this facility which include a medical and physical exam
\xe2\x97\x8fMust be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at time, hostile people within the facility.
\xe2\x97\x8fMust be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 3 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 10 feet.
\xe2\x97\x8fMay be necessary to assist in the evacuation of residents during emergency situations.
