: This position will be M-F roughly 730-8am start to a 1630-1700 end. This position is onsite only, no on call \xe2\x80\x93 no weekends no nights no holidays.This is for our neuromuscular ClinicThe Clinic Coordinator-Transplant facilitates effective clinic organization and patient care by serving as a communication link between the patients, staff, physicians, hospital departments or other outside health care entities. Ensures compliance to CMS requirements for communication to families and donors. Develops and maintains systems for data management to meet requirements and for reporting to internal and external consumers of the information. May perform a broad range of responsibilities including: receptionist, scheduler, patient registration, chart acquisition, and telephone triage. Must use critical thinking and problem solving skills and provide backup assistance as necessary to carry out daily operations.Job Essentials1. Clinic Scheduling: Ensures effective and efficient clinic flow. Coordinates clinic schedule by effectively scheduling appointments, collaborating with team members to maintain appropriate clinic load, and appropriately accommodating special needs. Identifies need and resources to accommodate expanded clinic time as necessary. Ensures effective appointment confirmation process. Assists with insurance prior authorization for procedures.
2. Call Scheduling: Responsible for entry and maintenance of on-call schedule for staff and surgeons.
3. Records/Charts: Establishes and maintains complete, accurate and timely medical records and other program documents in accordance with hospital, departmental, legal, and professional requirements.
4. Reporting: Develops and maintains systems for data management to meet requirements. Ensures accurate data entry for reporting to internal and external consumers of the information. Prepares quarterly QAPI reports to CMS, as well as, weekly selection meeting reports. Care process reporting to ensure patient care values including labs, test results and care steps are available to Nurse Coordinators and LIPs.
5. Patient Contact: Performs patient care duties including vital signs, heights, and weights within scope of licensure.
6. Correspondence: Creates correspondence to families, insurance providers, and other within requested timeframe and develops other program documents as required. Ensures compliance to CMS requirements for communication to families and donors.
7. Compliance/Quality: Ensures compliance with departmental, clinic and corporate policies, procedures and best practices. Identifies and appropriately manages compliance with regulations mandated by various federal, state or accrediting agencies. Identifies and implements best practices to ensure effective and efficient program operation.
8. Safety: Demonstrates focus on safety, adhering to all applicable safety standards including the National Patient Safety Goals and is pro-active in identifying and correcting unsafe practices and conditions.
9. Physician Relations: Demonstrates the ability to communicate effectively and work collaboratively with physician(s). Utilizes chain of command appropriately.
10. Supplies: Maintains an adequate inventory of supplies, office supplies, patient education materials and equipment to meet clinic requirements.
11. Community Stewardship/Education: Understands and actively engages in their assigned role for a disaster response. Is flexible with work schedule and assignments to support patient care service needs. May also participate and manage community functions and/or educational activities.Minimum Qualifications
