We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.The Clinical Data Coordinator is responsible for overseeing three key areas: healthcare quality data, metrics for HRSA and grant-related initiatives (such as Patient Centered Medical Home, UDS, site visits and FTCA), and compliance and incidence response and reporting. This role includes the added responsibility of serving as the Patient and Staff Safety Coordinator and will assist with various staff education related duties.Requirements:QualificationsEducation/Training/Licensure: Bachelor of Science in Nursing required, additional education in areas of responsibility preferred (but not required), such as an MBA in Healthcare Administration, or an MSN. Current New Hampshire RN License.Summary:Compensation: The starting rate of pay for this position is $34.50 per hour.
