Job Summary:
The Clinical Liaison will work closely with referral sources and the NCHHHA Intake team to identify prospective patients and provide them with details about NCHHHA\'s offerings and programs. The Clinical Liaison will round routinely at the North Country Healthcare affiliate hospitals and participate in meetings to assist with transitions of care to the home setting. Exceptional customer service and communication skills will be necessary for the Clinical Liaison to ensure referral sources and prospective patients needs are exceeded.

The requirements listed below are representative of the knowledge, skills and/or ability required. Minimum of 2 years experience in healthcare, home health and hospice experience preferred. Strong knowledge of home health and hospice and its relationship to the healthcare community. A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position. Access to operable, insured motor vehicle and valid driver\'s license with acceptable record of safe driving practice
