Location: Chatsworth by Erickson Senior LivingChatsworth is located in Palm Beach Gardens, Florida, on the grounds of prestigious PGA National. We offer a full continuum of care, exclusively for seniors, from assisted living to memory care and more.We are hiring a Clinical Liaison to join our Sales Team in our awarding winning Continuing Care Retirement Community (CCRC). As the Clinical Liaison, you are responsible for the data collection and coordination of admissions for referrals from inpatient acute care and sub-acute care settings, including internal residents of the Erickson Senior Living community. You will collaborate with Physicians, Continuing Care Sales/Admissions Staff, Hospital Case Managers, Social Workers, and/or Discharge Planners for referrals to facilitate the admissions/transfer process into our Assisted Living and Memory Care neighborhoods, to include Skilled Nursing when applicable.How you will make an impact:

• Perform onsite clinical collection of data of the referrals to determine appropriateness for transfer to Assisted Living, and Memory Care, as well as Skilled Nursing neighborhoods.
• Engage in marketing outreach to professional network of referral sources in hospitals, physician offices, senior care managers and other facilities.
• Perform discharge planning and care coordination for residents transferring from acute hospital to our Continuing Care neighborhoods to ensure a smooth transition.
• Provide tours for potential admissions, collaborate with families, and assist in the process of transitioning residents to a higher level of care.
• Identify quality issues and participates in quality management activities, as needed, with Administrator and/or Medical Director.

What we offer:

• Compensation: Starting at $69,000 plus commission, commensurate with experience.

What you will need:

• Bachelor\xe2\x80\x99s degree is preferred.
• 3+ years of clinical/healthcare experience in hospital and healthcare settings preferred.
• Liaison experience or healthcare marketing background preferred.
• Established relationships with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.
