:The Clinical Pharmacist will work collaboratively with primary care physicians, specialists, staff and leadership to develop programs that improve patient outcomes through appropriate medication management. Develop and implement methods to measure the results of these programs to justify expansion and drive quality improvement. Provide coordinate, comprehensive, high-quality care to patients. Responsible for the evaluation, interpretation and management of patient\'s medication therapies to improve the quality and outcomes of healthcare provided and control costs. Support clinicians and other health care providers through collaborative drug therapy management services, medication conversion programs and other functions to ensure that the use of pharmaceutical agents is rational and cost-effective. Provide direct oversight and supervision of Clinical Pharmacy Technicians. Primary responsibility for the processes of medication reconciliation, refills, and patient education on prescriptions.
