Community Healthlink (CHL) is looking for a full-time (40 hrs/wk) Clinician- Intensive Care Coordinator at our Leominster site.

Families and Communities Together (FCT) seeks to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities. FCT staff works with the family, child, appropriate state agencies and school staff to develop strategies that help improve child and family functioning.

Intensive Care Coordinators coordinate multiple services and supports for youth with Serious Emotional Disturbance (SED). The care coordinator facilitates the development of a Care Planning Team, assists the family and Team in identifying goals and developing an Individual Care Plan and Safety Plan. The Care Coordinator works directly with the youth and family to implement elements of the Plan, and coordinates with other Team Members involved with the family to help the youth and family reach their goals.

Responsibilities

  • Demonstrates a basic, functional knowledge of and endorses the CBHI values: Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative and Integrated, and Continuous Quality Improvement.
  • Can engage youth and their families quickly and develop strong working relationships with youth, families, Family Partners, and any other individuals who participate on a family\xe2\x80\x99s Care Planning Team (extended family, friends, other providers, school personnel, and/or state agency representatives)
  • Demonstrated ability to effectively communicate with youth, families, colleagues, and community members in a manner that is sensitive to their racial/ethnic/cultural/class backgrounds.
  • Maintains the highest standards of ethics and demonstrates professional attitude and conduct
  • Assist in the creation and maintenance of a positive and collaborative working environment where all FCT employees and enrolled youth and their families feel welcome, supported, and comfortable.
  • All activities described here will be delivered at the FCT offices, in the community, or in the family\xe2\x80\x99s home depending on the preferences of the enrolled youth and family.
  • Works to build strong, functional working relationships with enrolled youth and their families.
  • Conducts assessments of needs, values, and strengths in full partnership with the enrolled youth and family which includes regular administration of the CANS.
  • Works with each youth and family to assemble a Care Planning Team composed of family members, their natural supports, and, if applicable, state agency representatives, school personnel, providers, and other significant persons.
  • Coordinate and facilitate Care Planning Team meetings to develop an individualized Care Plan for each enrolled youth and their family which includes a Risk Management / Crisis Plan.
  • Works closely with the FCT Family Partner, the family, and the Care Planning Team to implement, monitor and modify the Care Plan as needed, and serves as an advocate/liaison for the family when appropriate to access new or different services.
  • Works in collaboration with the family to communicate with and involve other key individuals who have contact with the family such as teachers/school personnel, youth services workers, physicians, probation officers, lawyers, who may not be participating on the Care Planning Team.
  • Facilitates and monitors service authorization in collaboration with each family to assure compliance with Community Healthlink policies, procedures, and annual budget.
  • Prepares all necessary paperwork such as assessments (including the CANS), court reports, service authorization requests, Care Plans, Crisis Plans, contact notes, daily service logs, etc.
  • Participates as appropriate in all meetings, training, evaluation/research, supervision, quality management, and related activities as may be directed by the Supervisor of Intensive Care Coordination.
  • Follow all policies and procedures of the agency.
  • Work assigned hours, which may include some weekend and evening hours.
  • Carries an after-hours pager and provides telephonic and, at times, face to face after hours support to youth and families as directed by the Supervisor of Care Coordination.
  • Perform other related job duties as assigned by supervisor.

Qualifications
  • Grade C06: Clinician II- Intensive Care Coordinator: requires minimum qualifications of Bachelor\'s degree in psychology, social work, or related human services field
GradeC08: Clinician III: Intensive Care Coordinator: Master\'s degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts. Requires licensure as LMHC, LCSW, LMFT, or is actively working towards licensure in the Commonwealth of Massachusetts.

Grade C12: Clinician IV: Intensive Care Coordinator: Master\'s degree in a clinical course of study (social work, psychology, counseling or related field) and LICSW licensure or licensed Psychologist * Minimum of 2 years clinical experience with families and children
