The Community Health Navigator is responsible for providing non-clinical care coordination, health coaching, and/or navigation support to clients. Clients may be in recovery from, or at risk of developing alcohol and/or substance use dependence or have other chronic diseases such as Type II diabetes or hypertension. The Navigator will assist clients with accessing primary care, managing their chronic disease, and obtaining access to other community resources. The Navigator will assess client\xe2\x80\x99s health and social needs and work with clients to develop and implement individualized plans to support clients with meeting their health goals. The Navigator will have a pivotal role in assisting clients with addressing barriers to care, as well as providing health education and coaching to achieve improved client-level health outcomes.
REQUIREMENTS AND QUALIFICATIONS:
Entry level knowledge; Basic employment skills; High school diploma - GED equivalent or high school certificate
