This position will work with Public Health\xe2\x80\x99s Chronic Disease Prevention and Health Promotion (PHP) program to implement strategies outlined in their Community and Adolescent Health (CAH) program\xe2\x80\x99s Neighborhood Demonstration pilot project. The Neighborhood Demonstration Project leverages staff and partnership capacity from PHP to convene a coalition that represents neighborhoods affected by violence and other disparities (i.e., poor physical environment, social determinants of health, chronic disease, trauma, racism, disinvestment, over-criminalization, displacement, etc.). This position will support the pilot project partners, develop new activities and programs, help plan and implement community-based public health education and advocacy campaigns to increase access to pro-social engagement, improve community design by connecting safe and accessible places for people live, work, worship and play. This position is responsible for recruiting and bringing community members, leaders, organization and providers who serve them together to participate in the Neighborhood Demonstration Project, and help to develop culturally relevant strategies and activities designed to promote health and well being of community members as an upstream prevention effort. This position requires the knowledge, skills, and ability to work with and serve African American/Black community members in Multnomah County. It also requires person in position to continually integrate equity into professional practice, and to advance the Public Health Division priority of Disrupting Institutional White Dominance and Social Justice concepts through self-study and team activities. Activities include: Read, discuss, write, critical thinking, personal reflection, and the evaluation of new tools and data. Work with individuals and families who identify with the African American/Black communities and other diverse communities.
This position will work in the Gateway Children Center and routine telework.
As the Community Health Specialist 2, you will:
Maintain and develop relationships with community members, including youth and leaders from Black and/or African American communities and service providers
Coordinate planning processes with staff and community members to identify and prioritize community needs,and work with them to develop policy, systems and environmental change recommendations that address barriers that are shared by a community as a whole
Role model, encourage and motivate community participants and organizations to adopt and maintain healthful practices and lifestyles
Understand and educate others on the social determinants of health, racism as a public health issue, socio-ecological model and policy, systems, and environment change strategies
Serve as a liaison between upstream violence prevention partners and program staff, represent and advocate for the Program\'s priorities to be included in local policy initiatives
Assist with development and dissemination of culturally relevant public health messages in the media
Participate on relevant internal and external committees, coalitions and task forces, etc.

Minimum Required Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
Education/Training: Equivalent to completion of the twelfth grade AND
Experience: Two years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families; AND experience working with the community being served.

This position requires advanced knowledge, skill and ability and has experience working with the Black/African American community. Demonstrates an understanding of the traditions, values and customs of this community. Understands cultural/racial/ethnic factors, including systemic advantage or disadvantage, generational trauma, and any factors related to non-dominant culture affecting community health.
A valid BLS/ CPR card or the ability to obtain certification within 60 days of hire is required.

Community Health Worker Certification issued by the Oregon Health Authority
