Job Summary: The Hospice Continuous Care LPN is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family. The LPN also follows a comprehensive and responsive plan of care.

:

This position does not have supervisory duties

Provide direct skilled bedside nursing care to patients.

Ensure and document care and interventions provided as well as appropriate use of medication, durable medical equipment and supplies.

Qualifications:

NJ Nurse License required

Minimum one (1) year of clinical experience

Understanding of Hospice philosophy and needs of terminally ill

Strong problem solving and critical thinking skills

Excellent Communication Skills

Ability to travel within the state of NJ
