Job Description


Position Overview: The Community Liaison for Dementia Support will serve as a key resource in outreach, training, crisis stabilization, and memory screening within the assigned region. This role involves providing person-centered resources, developing partnerships, and offering education and consultation to various stakeholders. The position requires extensive travel to Northern Nevada counties and you will be provided a company car.Duties/Responsibilities:

  • Community Outreach and Training:
  • Serve as a community liaison for outreach, training, crisis stabilization, and memory screening.
  • Conduct outreach and education related to the Nevada Dementia Supports Toolbox and associated services.
  • Provide dementia-capability training to Nevada Care Connection Resource and Service Navigators and other service providers.
  • Client Support:
  • Provide person-centered resources and individualized referrals for individuals experiencing cognitive changes.
  • Conduct assessments and develop individualized care plans for clients.
  • Provide short-term case management for people with dementia and their caregivers.
  • Perform home visits as required.
  • Partnership Development:
  • Develop partnerships and referral pathways with emergency response personnel, healthcare providers, community organizations, family care partners, and people living with dementia.
  • Strengthen, grow, and support local Dementia Friendly Nevada efforts.
  • Documentation and Reporting:
  • Document case notes for each client interaction, including referrals, applications, and outcomes.
  • Ensure narrative notes include date of entry, summary of pertinent information, initials, and title of the person making the entry.
  • Advocacy and Education:
  • Serve as a visible and trusted source of information on long-term care options, referral, and advocacy.
  • Inform and educate consumers about their options and assist with linking them to programs and services.
  • Advocate on behalf of clients and their families with agencies and service providers.
  • Program Development and Marketing:
  • Participate in program development, marketing, and outreach activities.
  • Represent NVCC at public information fairs.
  • Schedule and attend outreach visits to offsite locations as needed.
  • Other Duties:
  • Perform other duties as assigned.
Other Requirements:
  • Ability to provide culturally competent and person-centered services.
  • Understanding of care partner and professional roles in supporting people living with dementia.
  • Compassionate and professional demeanor.
  • Knowledge of social services available in the community.
  • Ability to input critical information into databases quickly and accurately.
  • Clear and effective communication skills.
  • Critical thinking and resourcefulness.
  • Ability to manage conflict with coworkers.
  • Knowledge of Access to Healthcare Network Services and Systems.
  • Flexibility to adapt to changing work environments and duties.
  • Abide by HIPAA requirements.
Education Required:
  • Bachelor's (or Master's) degree in social work, health and human services, or equivalent experience.
Experience Required:
  • Valid Nevada driver's license.
  • Nevada Medicare Counseling certification within 4 months of hire.
  • Three years of experience in a social service or healthcare-related field.
  • Two years of experience working with socioeconomically disadvantaged populations.
  • Experience with the needs of older adults and their care partners.
  • Two years of experience with databases and software programs such as Microsoft Word, Excel, and PowerPoint.
  • Excellent organizational and customer service skills.
  • Excellent oral and written communication skills.
Experience Preferred:
  • Bilingual preferred but not required.
  • Four plus years working in a social service or healthcare-related field.
  • Three plus years working with socioeconomically disadvantaged populations.
  • One year paid experience working with people living with dementia and their families (or informal care partners).
  • Ability to recruit, host, and facilitate small and large group learning and discussion programs.
Physical Requirements:
  • Ability to stand between 3 - 6 hours per day.
  • Ability to sit at a computer or in a car for up to 4 - 6 hours per day.
  • Ability to drive for up to four hours at a time, if necessary.
  • Must be able to lift and carry up to 30 pounds consistently.
  • The noise level in the work environment is usually moderate, and the employee typically works in a climate-controlled office environment.
