Job Description

JOB SUMMARY: This non-exempt management position, in cooperation with the Services Director, is responsible for ensuring people receiving waiver services in a residential and community setting are provided support which promotes dignity and respect; is individualized according to the person's preferences, strengths and needs; and creates opportunities for meaningful relationships and community involvement. This position provides service coordination for individuals living in their own home. This position is also responsible for ensuring that policies, procedures, and practices comply with applicable state and federal law and are aligned with Frontier's commitment to person-directed services.

o Requires demonstrated proficiency with computers, and knowledge of basic software applications utilized for word processing and excel.

ESSENTIAL FUNCTION:

COORDINATION DUTIES

o Provide case management assistance in collaboration with the Individual(s) reflecting the individual's choices and preferences for the service being provided. Examples of this may include coordinating support and access of medical, education and vocational services or obtaining documents such as a social security card.

o Know and understand Assisted Living Home licensing regulations and ensure the home maintains compliance with licensing if applicable.

o Schedule, plan, and provide services as outlined in the Individual's plan of care and bill for the service.

o Schedule and orchestrate bimonthly home visits with clients on caseload to assess case management and service needs.

o Assist clients in identifying, accessing, and coordinating services outside of FCS Waiver program, including monitoring of services provided and follow-up communication with service providers.

o Complete annual assessments as needed based on state, grant, and COA standards and requirements.

o Complete supply order inventory, grocery shopping, credit card reconciliation, and consumer funds requests as needed for homes/clients identified on caseload.

o Complete client education and meet and greets for new hires as applicable to caseload.

o Conduct trainings with clients for Direct Support Professionals on a backup basis, as needed.

o Monitoring the implementation of services, including encouraging and motivating clients to engage with providers and participate in their own care/services management.

o Take clients to medical appointments as directed by the Medical Coordinator.

o Ensure documentation of all significant events and activities are maintained in accordance with agency policy.

o Convene and facilitate person-centered service team planning meetings.

o Develop and update as needed information in Alayacare including service goals and objectives.

o Monitor health and safety of clients, making necessary reports to appropriate person or persons, i.e. Critical Incident Reports, Adult Protective Services Reports, etc.

o Respond to individual and/or team member service-related concerns.

o Participate in transfer meetings pertaining to newly referred individual(s).

o Participate in community events to promote FCS Services on an as needed basis.

ADDITIONAL JOB DUTIES:

Attend all required trainings and meetings.

Participate in agency-wide quality improvement, including developing ways to measure. performance, collecting data on performance and using the data to improve service delivery.

Conduct and oversee quarterly file reviews and monthly data reporting.

Other related duties as assigned.

Job Type: Full-time

Pay: $22.21 - $25.58 per hour
