The Discharge Planner/Clerk provides duties including: completion of Social Assessments, aftercare appointments, and referral communication. The Discharge Planner/Clerk works within set guidelines and under direct supervision to perform simple and repetitive clerical related tasks including but not limited to: printing, basic filing, copying, opening and sorting of documents, transcribing patient information, taking notes for therapists, organization of files/charts, and other duties as assigned. Knowledge of general office practices. Ability to maintain clerical records and operate basic office equipment. Ability to interact on the telephone or in person with the public/patients/staff as this position interfaces with referral sources, community service agencies, and businesses. Ability to receive/accept direction from licensed clinical staff. This position also ensures compliance for quality patient care and compliance with Joint Commission standards, State Health Department requirements and all other regulatory standards.
