Social Service Specialist 3

The has an excellent opportunity to join our team in Spokane, WA. This is a specialist-level social service role within the Behavioral Support Team at Home and Community Services (HCS) working with Medicaid clients who live in residential settings such as Adult Family Homes (AFH), Assisted Living Facilities (ALF), and Enhanced Services Facilities (ESF) and who may experience complex and challenging behavioral symptoms with periods of instability. The population served by this role are also high-risk, frail, vulnerable persons aged 18 years and older who are medically and financially dependent on services from Home and Community Services.

This role works effectively and collaboratively with our community residential facility partners, acute hospitals, state hospitals, behavior support providers, Managed Care Organizations, evaluation and treatment facilities, and other community psychiatric settings, fostering good communication and demonstrating reliability.

This role is effective at utilizing all appropriate and available resources such as Expanded Community Services, Specialized Behavior Support, Specialized Pilot Programs, Foundational Community Support, GOSH, Washington Roads Expansion - Community Choice Guide, Challenging Behavioral Support to assist in promoting stable community settings and resolving issues and concerns.

The Social Service Specialist 3 role conducts comprehensive assessments at the client\'s residence or other settings as approved, using a laptop computerized system to determine functional eligibility for Medicaid long-term care services and determination of the level of care. This role also develops person-centered and individualized service plans in conjunction with the client and/or the client\'s representative to meet the needs of the client

Some of what you will do:

  • Conduct functional assessment utilizing the department\'s Comprehensive Assessment Reporting & Evaluation (CARE) computerized tool to determine level of care and eligibility for the Residential Support Waiver (RSW), Community Options Program Entry System (COPES), Medicaid Personal Care (MPC), Community First Choice (CFC), and other programs administered by Home & Community Services for initial or on-going service coordination in residential facilities such as Assisted Living Facilities (ALF), Adult Family Homes (AFH) and Enhanced Services Facilities (ESF)
  • The assessment will occur in a clinical setting, a client\'s residential facility, a client\'s home, or another approved setting
  • The CARE assessment will be completed by meeting the quality standards as set forth in the Long-Term Care Manual
  • Develop appropriate individualized service plans in conjunction with the client and/or representative to meet the needs of the client
  • Authorize appropriate services for Medicaid clients utilizing qualified providers if the client chooses to receive their long-term care services in a residential setting or an in-home setting
  • Collaborate with local residential facilities, contracted providers, MCOs, and possibly, discharge planners to ensure a continuation of care to meet the client\'s needs Provide skilled ongoing case management services to resolve issues, and concerns and provide advocacy and support to divert or prevent clients from admitting to hospitals or skilled nursing facilities

Who should apply?
Professionals with one of the following:
  • One year as a Social Service Specialist 2
OR
  • A Master\'s degree in social services, human services, behavioral sciences, criminal law/justice, or an allied field and one year of paid social service experience equivalent to a Social Service Specialist 2
OR
A Bachelor\'s degree in social services, human services, behavioral services, criminal law/justice or an allied field, and two years of paid service experience performing functions equivalent to a Social Service Specialist 2

Please note: A two-year Master\'s degree in one of the above fields that include a practicum will be substituted for one year of paid social service experience.
