The Southcentral Foundation (SCF) Elder Resource Specialist is responsible for partnering with assigned Elders and caregivers to provide information, assistance, and referral to community resources, transportation to medical appointments, essential services, and in-home or group visit services as defined in their service plan with the goal of keeping Elders living independently for as long as possible.
The Elder Resource Specialist demonstrates the ability to collaborate with customer-owners, clinical staff within the SCF Primary Care Center and Alaska Native Medical Center Campus and other local medical facilities, municipal, state, and federal agencies and private legal businesses within the community regarding the synchronization of a broad range of services and resources as well as time-intensive care coordination tasks.
This position has two (2) levels designed to provide progressively more responsible and independent work experiences. Progression between job levels is based on the demonstrated ability to successfully handle more progressively responsible assignments.
Qualifications:
SCF programs are established to serve a primary population comprised of Alaska Native people who are affiliated with Cook Inlet Region, Inc. (CIRI) and Alaska Native and American Indian people within SCF\xe2\x80\x99s geographical service area. Employees should have a thorough understanding of the cultures and the needs of this population. Such knowledge is critical to ensure the achievement of SCF\xe2\x80\x99s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness, and mission of working together with the Native Community to achieve wellness through health and related services: * Associates degree in Health Care, Human or Social Services, or closely related field from an accredited college or university; OR equivalent combination of education, training, and experience.
