Nature of WorkUnder general supervision, the Health and Human Resources Associate, HealthCheck Associate, is assigned to approximately 1500 Medicaid eligible West Virginia children ages 0 to 20 which are not enrolled in a Managed Care Organization (MCO). This position is an outreach specialist position that will inform and support services to Medicaid eligible children per requirement of the Early and Periodic Screening Diagnosis and Treatment (EPSDT) services. The selected candidate will work closely with HealthCheck and programs within the Infant, Child, Adolescent and Young Adult Health (ICAYAH) Division.The incumbent utilizes a combination of written and oral methods to effectively inform caregivers about the benefits of preventive health care, services available through EPSDT without charge and support services, specifically transportation and appointment scheduling assistance. Support the medical home approach to providing comprehensive primary care for children by ensuring that his/her assigned clients have an identified physician or primary care provider (PCP) who provides first contact care. Incumbent facilitates appointment scheduling for appropriate periodic examinations for each month in accordance with the American Academy of Pediatrics (AAP) periodicity schedule.
