He or she will be responsible for managing schedules, coordinating care, and ensuring that our clients receive the highest level of service possible. Serve as a liaison between patients, families, and the interdisciplinary care team.
Participate in patient and family conferences to develop individualized plan of care
Monitor the delivery of care and services rendered by home health aides and other staff members to ensure compliance with physician orders and agency policies and procedures
Make frequent visits to patients\xe2\x80\x99 homes to observe aides\xe2\x80\x99 performance and to assess patients\xe2\x80\x99 condition and progress
Investigate and resolve complaints made by patients or their families regarding the quality of care received
Keep accurate records of patients\xe2\x80\x99 medical histories, treatments, and progress
Prepare reports on patients\xe2\x80\x99 progress for physicians and other members of the healthcare team
Supervise and provide in-service training to new aides and other staff members
Maintain contact with community resources to ensure that patients have access to all available services
Develop and implement plans for the continuity of care of patients after they are discharged from the home health agency
Participate in quality assurance activities to help improve the overall level of care provided by the agency
Perform administrative duties such as preparing budgets and scheduling staff.
Required Skills and Qualifications
Home Care business administration experience, or related field would be preferred.
3+ years of experience in a home care management role (preferred)
Proven track record of increasing revenue and profitability
Strong understanding of state and federal regulations governing home care agencies
Excellent communication, interpersonal, and leadership skills
Demonstrated ability to develop and implement policies and procedures
