Seeking Home Health Licensed Vocational Nurses in the Lewisville, Carrolton, Flower Mound and Coppell, TX area. We are also hiring LVN/PRN\'s for Weekdays (Monday-Friday).

Benefits-Bonus-$15,000 (Full-time only)

Pay rate: $35-45 pr visit

Responsibilities

• Skilled Visits
• Assists in development, review and revision of the plan of care.
• Provides quality patient care set forth by the RN Case Manager\'s Care plan
• Observes, records and reports reaction to treatment and any changes in client\xe2\x80\x99s condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.

Requirements

• Home Health (Geriatrics) experience
• Texas Nursing License
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver\xe2\x80\x99s license
