Responsibilities

• The HCP of Texas Registered Nurse Case Manager is responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care.
• RN Case Manager will partner with an LVN and have an assigned territory within a manageable proximity.

Requirements

• Home Health (Geriatrics) experience
• Active RN License in the state of TX
• Knowledge of Wellsky (Kinnser)
• Driver's License and Vehicle with Current Auto Insurance
• Problem solving skills and ability to multi-task
• Excellent Communication Skills
• Positive, Professional attitude
