Description
A Medication Reconciliation Resource Nurse provides indirect patient care by ensuring that the medication reconciliation process is completed per Policy and Procedure at the time of admission, at the time of transfer in level of care and at the time of discharge. This RN will interact collaboratively with the patient, family, staff, physicians, pharmacists, and any other resource needed to effectively complete this process according to the National Patient Safety Standards. This RN will also assist in ensuring the quality of medication reconciliation by working closely with staff nurses and Unit Directors.
KEY RESPONSIBILITIES:
1. Demonstrated leadership ability and demonstrated ability to work with interdisciplinary team effectively. 2. Ability to work with and direct others in the care of patients within a stressful environment by setting priorities making decisions and delivery of appropriate care.
3. Thorough knowledge of hospital policies, procedures and standards of care.
4. Thorough knowledge of medication reconciliation policies and procedures demonstrating excellent interviewing skills with patients and families with ability to assist other staff members and physicians.
5. Willingness to maintain current knowledge and skill related to the care and support of patients normally cared for on the assigned nursing unit.
6. Proficient computer skills.

MINIMUM EDUCATION REQUIRED:
Graduate of an accredited school of nursing.
MINIMUM EXPERIENCE REQUIRED:
At least 2 years experience in Inpatient Nursing Unit
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Registered Nursing license with State of Georgia.
