The Hospice Admissions Nurse is responsible for providing a compassionate and thorough assessment of patients referred to hospice care. They serve as the first point of contact for patients and families, ensuring that all necessary clinical information is gathered and communicated and that patients are fully informed about their care plan. The nurse collaborates with interdisciplinary teams to facilitate smooth and timely admissions and provides education on hospice services to patients, families, and other healthcare providers.Desired Experience:
Bachelor of Science in Nursing from an accredited program in Nursing required
Minimum (2) two years\xe2\x80\x99 experience in Hospice with (5) five years\xe2\x80\x99 experience in nursing
Current license to practice as a Registered Nurse in Connecticut.
Certification in Hospice and Palliative Care preferred, required within 2 years of hire
Computer skills
Ability to identify needs in workflow process
Knowledgeable and compliant with all agency and regulatory procedures including, but not limited to, Corporate Compliance, HIPPA and Medicare conditions of participation, State of CT Licensure, and CHAP Standards.
