The Hospice Clinical Leader / Hospice Clinical Educator is a hybrid role designed to provide leadership, clinical support, and education to both new and existing hospice staff. This position involves a balance of office-based work, including overseeing clinical operations, staff development, and policy updates, and field-based work, where the leader/educator will mentor and support clinicians in real-time patient care. The role ensures that hospice staff are aligned with best practices, regulatory standards, and the organization\'s mission of providing compassionate, high-quality care.
Bachelor of Science in Nursing from an accredited program in Nursing required
Minimum (2) two years\xe2\x80\x99 experience in Hospice with (5) five years\xe2\x80\x99 experience in nursing
Current license to practice as a Registered Nurse in Connecticut.
Certification in Hospice and Palliative Care preferred, required within 2 years of hire
Computer skills
Ability to identify needs in workflow process
Knowledgeable and compliant with all agency and regulatory procedures including, but not limited to, Corporate Compliance, HIPPA and Medicare conditions of participation, State of CT Licensure, and CHAP Standards.
