An Overview
The Human Services Specialist II, IDD Intake/Eligibility Waitlist Coordinator conducts initial intakes and provides Medicaid Waiver services to children and adults with Autism, Developmental or Intellectual disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. The IDD Intake/Eligibility Waitlist Coordinator duties relate to completing intake and eligibility screenings from multiple referring agencies and individuals and monitoring the status of individuals on the Medicaid Waiver Waitlist. This position may be trained in multiple aspects of the job to allow for backup support within the unit and agency.
The Human Services Specialist II, IDD Intake/Eligibility Waitlist Coordinator position requires frequent use of initiative and independent judgment; professional courtesy; in person, intermittent short-term contact with individuals, families and service providers and the ability to work with minimal supervision. Job responsibilities may require traveling within the service area and flexible hours and schedule.
Essential duties include completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage, monitoring and advocating on behalf of individuals, documentation of services provided and interaction with the individual/family served.
An employee in this class receives oversight and case review from a supervisor. Client interactions are sensitive and require that employees maintain confidentiality of client records and treatment. The work performed in this classification does not require clinical licensure.

The ideal candidate will have knowledge of available community resources, such as medical, legal, mental health and, economic and social services; Knowledge of treatment issues related to behavioral, emotional, economic, and social conditions, including dual diagnosis; Skill in identifying and linking individuals to supports and resources; Skill in engaging diverse stakeholders and in communicating clearly and effectively, both orally and in writing; Ability to evaluate client needs and formulate outcomes to resolve resource needs; The ability to provide competent, professional work with culturally diverse families.
