Summary: The Home Infusion Therapy Registered Nurse is responsible for the nursing needs in the home infusion department. The RN sets up all new patients and coordinates with the Yorhom insurance verification and billing departments. The RN teaches patients on the use of home infusion equipment and supplies, including troubleshooting and reordering of supplies. This position rotates on-call with other RNs and the home infusion PharmD for maintenance and troubleshooting. The Home Infusion Therapy RN also educates and sets up patient tube feeding needs.
Essential Job Functions:
Participates in and carries out initial home set-up visits as assigned.
Assesses patients for appropriateness of admission according to HIT criteria. Conducts insurance screening for patients referred to HIT program. Identifies physical and psychological symptoms and changes during each visit and updates care plans as appropriate.
Implements components of the patients care plan and treatment. Develops and updates individualized care plans on assigned patients. Makes routine contacts to patients to reassess patient status, evaluates clients response to care, monitors supply and equipment management and updates the care plan.
Collaborates and communicates with appropriate individuals to collect data and identify needs of potential Home Infusion patients and ensure timely and efficient discharge planning from inpatient to admission to Home Infusion Therapy (HIT).
Carries out established techniques for safe administration of medications and treatments in accordance with HIT policies and procedures. Ensures up-to-date documentation and medical records are maintained on all patients.
Participates in Quality Assurance activities as assigned. Participates in program expansion/diversification activities as assigned. Responds appropriately to emergency situations in accordance with HIT policies and procedures.
Utilizes patient teaching opportunities and organizes and implements pre-admission teaching for new HIT referrals.
Performs pain assessment and takes appropriate action according to standard operating procedure.
Performs other duties as assigned or needed to meet the needs of the department/organization.
