Summary: The Inpatient Pharmacist is an essential members of our health care team and interacts with patients, providers and all other team members to deliver medications safely and effectively to our patients. In this inpatient setting, the pharmacist participates in a wide variety of patient care activities including; medication management, patient rounding, kinetic monitoring, dosing of medications and sterile and non-sterile compounding. This position serves as a readily accessible resource for our patients and other team members. The Inpatient Pharmacist engages and interacts with patients on a daily basis.
Essential Job Functions:
Demonstrates the ability to accurately interpret, manufacture, and dispense compounds, extemporaneous pharmaceutical products, intravenous admixtures, small volume parenteral, total parenteral nutrition and chemotherapy orders.
Interprets verbal and written medication orders, checks for patient name, location, drug name, route of administration, appropriateness of dose and frequency of administration.
Enters medication orders into the computer, reviews and monitors patient\xe2\x80\x99s past and present medication records, checks for duplication or medication errors, incompatibilities, adverse reactions and drug interactions. Completes all required documentation.
Provides educational information and programs to patients, families, health care providers and community organizations, i.e., physician rounding, in-services, consultations.
Communicates with other caregivers regarding the patient\xe2\x80\x99s status and needs.
Maintains appropriate patient and medication records in compliance with state and federal regulations and department policies and procedures.
Instructs and supervises order entry functions performed by technical support. Maintains adequate stock supplies, usage trends and anticipates stock levels.
Participates in creation, revision, and evaluation of Quality Assurance indicators.
Performs pain assessment and takes appropriate action according to standard operating procedure.
Performs other duties as assigned or needed to meet the needs of the department/organization.
