Position:Integrated Health Case ManagerDivision:HealthDepartment:Behavioral HealthReports To:Integrated Health Case Manager SupervisorLocation:LincolnExempt/Non-Exempt:Non-ExemptSalary:Tier 1: Grade 6 ($$15.56-$24.94)Tier 2: Grade 7 ($17.52-$26.70)Classification:Full-timeSummary:The Ponca Tribe of Nebraska is a federally recognized Tribe with its ancestral homeland located in Niobrara, NE. The Tribe does not have a reservation in the classic definition; rather, the Ponca Restoration Act defined its jurisdiction as a \'Service Delivery Area\' comprised of 15 counties throughout parts of Nebraska, Iowa, and South Dakota. Within its Service Delivery Area, the Tribe offers a broad range of health, social, educational, and cultural services.This position will provide person centered support to individuals receiving mental health and substance abuse counseling with an emphasis on culturally sensitive interventions to individuals and families. In addition, serve as a liaison and provide additional support to others involved in client\'s treatment, including, but not limited to family, employers, schools, probation/parole, courts, primary care team, etc.Essential Responsibilities and Duties: * Using a motivational, staged approach, to provide individualized care based upon strengths, needs, abilities, and preferences identified collaboratively with client/family, focused on supporting client\'s movement toward recovery/resiliency goals.

  • Assist client in identifying strengths, needs, abilities, and preferences in regard to mental illness/substance abuse and participate in development of care/wellness plan.
  • Provide services from a holistic perspective, incorporating coaching, with attention to each domain of wellness, including physical and mental health.
  • Communicate actively with other members of the integrated team, including psychiatrists, primary care provider team, specialists, dentists, regarding all areas of client wellness, including client\'s goals change.
  • Make referrals to additional services/providers as needed for client\'s overall wellness.
  • Support client in completion of goals identified on care/wellness plans. This will include reviewing the plan with client and updating of goals and objectives as items are achieved, or as the client\'s goals change.
  • Aid in accessing public services, including financial, housing, emergency assistance, food, etc.
  • Maintain contact with clients who are hospitalized, participate in discharge planning, and communicating with all involved providers in developing aftercare plans.
  • Provide education and skill-building in areas such as daily living skills, employment/education, symptom management, communication/relationships, budgeting, maintaining a residence, etc., with focus on movement toward independent completion of these tasks, or referral to long-term supports
  • Assist clients with developing personal support systems, including, but not limited to, work with family members, legal guardians, recovery communities, etc.
  • Provide individual assistance to community members and clients in accessing needed mental health services including accompanying clients to appointments to address medical and/or other health needs.
  • Follow-up with clients regarding appointments, completion of forms, and other similar activities.
  • Maintaining Ponca Pantry, which includes shopping, inventory, assessment of clients, and coordination on day of pantry.
  • Will follow the policies and procedures of Ponca Health Services and the Behavioral Health Department
  • All data recorded will be timely; accurate; and current.
  • Patient confidentiality and Privacy Act/CFR 42 and HIPAA must be enforced.
  • Other duties as assigned, including but not limited to: assisting with training new team members, as appropriate, assisting with grant tracking, assisting with grant data collection, attending grant recipient award meetings, and youth group.
  • Additional training, certification, and compensation for provisionally and/or licensed drug and alcohol counselor may be available based on employee interest and clinic need. This would include conducting chemical dependency evaluations and assisting with group substance use treatment.
Relationships and Authority:This position has no supervisory duties.Working Environment and Physical Demands:This position is frequently driving clients and operating in the community. Sits, stands, bends, lifts, twisting, and moves intermittently during work hours. Regularly lifting up to 100 pounds and occasionally lifts more. Is subject to emotionally upset patients, family members, personnel, and visitors. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, unreceptive people.Additionally, there will be time spent in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, scanners, and photocopiers.Qualifications:Minimum:
  • High school diploma or GED.
  • Experience with maintaining client documentation in an Electronic Health Record.
  • Computer proficiency in Microsoft Word, Excel, and PowerPoint programs.
  • Excellent written and oral communication skills.
  • Valid driver\'s license and be insurable.
  • Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
  • Able and willing to travel.
Preferred: * Bachelor\'s degree in Social work or Health Related field.
  • One year experience working with Native Americans or Native American organizations.
  • Knowledge and understanding of Public Law 93-638" Indian Self-Determination Act.
Tier 1: (Grade 6) * High school Diploma or GED
  • 1 years\' experience working with individuals with mental health/psychiatric disorder and/or substance use disorder.
Preferred: * Associates Degree in Social Work or Health Related FieldTier 2: (Grade 7) * Bachelor\'s Degree in Social Work or Health Related Field
  • 2+ years\' experience working with individuals with mental health/psychiatric and/or substance use disorder.
Preferred: * Master\'s in Social Work and eligible to be Certified Social WorkerPHS Category:Designation:Specify:Risk Exposure LevelLow/Moderate/HighModerateAnnual CredentialingYes/NoNoIT PrivilegeAprima/Dentrix/ScriptPro/OtherAPRIMAPreference will be given to Ponca Tribe of Nebraska members or members of a federally recognized tribe.
