JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being effective team player. Assist in the development and implementation of Quality Improvement (QI) activities including data collection, analysis, and reporting within the Intellectual Disabilities Department. Identify and conduct compliance, fiscal and staff service documentation audits. Work collaboratively with other departments within the agency. Must be highly organized with good analytical and problem-solving skills. Use of personal vehicle for travel to off-site locations within 15 minutes of main campus at 8220 Castor Avenue.

QUALIFICATIONS:

  • Bachelor\xe2\x80\x99s degree in Human Services, Special Education, or related field
  • Experience with individuals with special needs; experience in continuous quality improvement and service documentation preferred.
  • Excellent computer skills required in Microsoft Office, including Excel, Word and Microsoft Outlook.
  • Knowledge of program evaluation, continuous quality improvement, and managed care procedures related to the intellectual disabilities field.
  • Excellent writing and analytical skills required.
  • Strong attention to detail.
  • Driver\'s License required.
SPECIFIC DUTIES:
  • Work with multiple Intellectual Disabilities Programs i.e. Community Living Arrangements, Pre-Vocational Habilitation Program, Community Integrated Employment, and Adult Day Program to ensure adherence to compliance standards as set forth by the state of Pennsylvania.
  • Maintain a high level of organization with ability to negotiation multiple demands and independently follow up on projects/tasks.
  • Audit ID service documentation (Monthly Progress notes, Quarterly Reviews, and Annual Assessments, etc.) to ensure compliance with PATH Documentation Policies and Procedures in conjunction with the state mandated regulations and guidelines.
  • Independently maintain running records of service documentation reviewed and future record assessment for the purpose of in-house reporting and follow up.
  • Assist in the collection and organization of service documentation necessary to meet the requirements of the Provider Monitoring Process at the discretion of the ID Support Supervisor.
  • Be familiar with the PATH Quality Improvement Plans for intellectual disabilities services and help support implementation and analysis.
  • Assist in development, implementation, and analysis of satisfaction surveys distributed to program participant families.
  • Coordinate and conduct outcome interviews for intellectual disability programs, including preparation and distribution of feedback reports.
  • Provide training in conjunction with ID Support Supervisor to other staff members regarding service documentation; policy changes and in house project management.
  • Serve as co-host with ID Support Supervisor for development of agenda items and presenting of meeting topics for the ID Consumer Advisory Committee.
  • Be familiar with continuous quality improvement literature and information with specific attention to intellectual disabilities services.
  • Serve as back up to the ID Support Supervisor in the agency Safety Committee, providing input for quality improvement activities.
  • Develop and maintain computer skills related to data entry and analysis, including the agency MIS system. Be proficient with Microsoft Office products, including Excel, Word. In addition, learn new agency systems such as Avatar i.e. Electronic Health Care Records for the purpose of training and helping coworkers.
  • Assist in development, revision, and maintenance of ID policies and standard operating procedures.
  • Prepare written reports as directed.
