- Facilitate referrals to community resources and refer to case management services.
- Conduct psychosocial assessments to identify patients' needs and formulate treatment plans.
- Document and maintain clinical records per appropriate standards and regulations.
- Participate in clinical supervision, team meetings, and trainings.
- Licensed LCSW in the state of California.
- Masterxe2x80x99s degree in social work or psychology.
