This position is responsible for the provision of family-centered culturally competent services for children and their families, birth to eight with typical development and developmental delays. The individual uses a primary service provider and coaching approach to instruct families and their children in activities designed to promote social, physical and intellectual growth needed for child development in their natural environments. This position provides interim and on-going services to families receiving services from Imua Family Services. The individual compiles the necessary information and data for programming and to create accurate reports and billing for various funding sources. Essential duties and responsibilities also include providing information to other professionals and the community as well as conducting other clerical work and administrative duties as required.**In addition to standard Care Coordinator Duties and Responsibilities, the LCSW/LSW will play a crucial role in helping children birth to six enhance their capacities and coping mechanisms, improving their daily lives and experiences. Within specified standards and timescales, the LCSW will provide valuable support and guidance, fostering positive change. The LCSW/LSW is responsible for providing initial evaluations, individual, group, and/or family therapy. In addition, the LCSW will provide consultation services to other team members. The LCSW will focus on a parent coaching model to support caregivers in helping their child reach their developmental milestones. They will be responsible for providing home and community based early intervention services to children birth to six. They will partner with parents/caregivers to ensure services are specifically addressing the priorities identified by the family. They are part of a transdisciplinary team providing home-based assessment, treatment, support and education services to families with children birth to 6 years of age.**Minimum Qualifications * Bachelor's or Master's Degree in the individual's specific discipline filed (e.g. Social Work) from an accredited College/University required.
