Lead Care Coordinator

(Full Time; 40 hours/week)

Generations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.

JOB SUMMARY:

Lead Responsibilities:

  • Training of new employees.
  • Training existing employees on new procedures.
  • Monitoring and reporting on existing systems within the department.
  • Assist supervisor with implementation of new systems.
The Lead Care Coordinator will conduct comprehensive biopsychosocial assessment to gather in-depth information about a patient/family\xe2\x80\x99s situation and functioning to determine patient needs, based on risk level identified by the patient\xe2\x80\x99s provider. Provider recommendations will be used to determine the level of intervention. Establish specific objectives, goals, and actions designed to meet the patient/family\xe2\x80\x99s needs as identified through the assessment process. Work collaboratively with the client to develop a comprehensive patient focused care plan that will address those needs. Coordinate and organize activities planned to help enhance client\xe2\x80\x99s access to health care and community resources and reduce risk behaviors. Involve clients in planning and assuming responsibility for such activities. Systematically maintain on-going contact with client per program protocol to determine progress and need for continuing support. Provide home visits to clients as appropriate per agency protocol. Work independently to assure reduction in barriers to healthcare access and to understand child development, healthcare necessity, parenting issues, etc. Coordinate and modify care plan, activities and/or interventions as required to continue accomplishment toward goals set forth in the plan. Provide information and explain community resources available to client as appropriate. Provide linkages with specialists and other health and human services providers to meet the identified as may be required. Serve as an advocate on behalf of patient and families accessing health care and other human services including enrollment/linkages to resources. Provide application assistance and act as liaison to enroll eligible participants in applicable programs, etc., as needed. Troubleshoot problems and bring unresolved problems to attention of supervisor or provider on a timely basis for consultation and resolution. Complete necessary documentation associated with caseload, including case plans, social histories, etc. Conduct outreach to inform community and providers of services. As directed, attends and conducts promotional activities as representative of Generations Family Health Center, Inc.

Qualifications

Bachelor\xe2\x80\x99s degree in human services or related field or equivalent related course work and work experience desired. Minimum of two (2) years of case management experience. Bilingual ability (English/Spanish) may be preferred based on service area and population served.

Pay Rate: $20.25/hour
