Our MissionTo improve the health of the communities we serve through contemporary, innovative, quality healthcare solutions.What You\'ll DoFulfill five-year Primary Care Training and Enhancement \xe2\x80\x93 Residency Training in Mental and Behavioral Health (PCTE-RTMB) grant project goals and objectives by participating in a multi-disciplinary health care team in service delivery for patients at the Continuity of Care Clinic. Responsible for coordinating care for all patients and assuring communication between the clinical team, patients, and their families.RequirementsCurrent Missouri licensure as a Licensed Clinical Social WorkerPreferred RequirementsMaster\'s Degree in Social WorkWork experience with behavioral health social work preferred.Work experience with critical care environments strongly preferred.Freeman Perks and Programs
