Overview

Min Rate = $22.88 / Max Rate = $33.18 *Placement is in accordance with the Collective Bargaining Units Agreement.

The LPN works under the general supervision of the Practice Supervisor, an Ambulatory RN or other licensed provider to deliver excellent patient care completing the duties and responsibilities of the position while complying with the HIPAA and CVPH confidentiality policies. S(he) is responsible to manage patient flow while supporting physician (s) to ensure efficient use of their time and minimize patient wait time. S(he) is responsible to perform operational and quality/risk management duties according to regulations and protocols. The position requires regular use of independent judgment to interpret and apply policies, procedures and practices.

Responsibilities * Under the direction of a RN or other licensed provider. Duties include but are not limited to:

  • Provide patient documentation including printing of test results
  • Provide wound evaluation, care and education including wound vac management, stoma care, dressing change, removal of staples and sutures
  • Coordinate with outside agencies to provide continuity of care as needed
  • Call patients with lab & pathology results
  • Maintain surgical instruments , including proper care & handling
  • Support In- Office Procedures including but not limited to:
  • Set up and cleaning of procedure room and sterile field
  • Assist physician(s) with surgical procedures as needed
  • Prepare and deliver specimen to the lab
  • Assist with patient calls to include:
  • Gather patient information to aid in decision making
  • Communicate patient concerns to RN or other licensed provider
  • Prescription refills and third party authorizations
  • Returning patient calls as needed
  • Manage patient flow including but not limited to:
  • Prepare exam rooms for patient visits
  • Place patients in exam rooms
  • Conduct patient interview (including meaningful use requirements)
  • Determine reason for visit
  • Obtain health history including meaningful use requirements
  • Document medications and medication changes
  • Identify and conduct appropriate intervention for suspected domestic violence and adult abuse
  • Review care plan with patient and reinforce education and necessary follow-up
  • Documentation of nursing care provided according to identified standards of practice
  • Maintain medical supplies to ensure efficient patient flow with cost containment in mind
  • Establish and /or maintain a safe work environment according to OSHA, federal and New York State guidelines
  • Monitor emergency equipment
  • Maintain infection control practices for medical equipment including cleaning and maintenance of equipment
  • Controls and protects medical practice assets and resources
  • Collaborate with Practice Coordinator to problem solve QA/RM issues
  • Maintain skill competency.
  • Demonstrate commitment to continuing education.
  • Other duties as assigned, including accompanying physician(s) on rounds to facilitate continuity of care and back-up to front desk as needed.
Qualifications * Graduate from an accredited School of Nursing required.
  • Currently licensed as a Licensed Practical Nurse in New York State.
  • Understanding and commitment to CVPH core values of teamwork, commitment, confidentiality and quality care.
  • Excellent attention to detail.
  • Good written and verbal communication skills.
  • Above average spelling and memory skills.
  • Working knowledge of medical terminology and medications.
  • Develop and maintain effective relationships with staff, providers, patients and external customers.
  • Medical office experience preferred.
  • Knowledge of medical practice management and/or electronic medical records systems preferred.
  • Utilize available business technology to perform assigned duties.
  • Above average organizational skills.
  • Ability to make appropriate decisions with a minimum amount of supervisory direction.
  • Experience in use of personal computers required; proficient in use of Microsoft Word.
As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
