$5,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.

The LPN / LVN participates, coordinates and provides direct nursing care in a manner which ensures participant\'s satisfaction and retention. Assists with medication orders and delivery, coordinating with the physician, RN, transportation and home care. Assists physicians and nursing staff with a variety of tasks, including but not limited to vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture and micro coagulation, catheter care foot care, administration/reading PPD tests and participant set up for physician visit. Performs duties in a variety of settings including the community, the Center, and the Clinic. May be asked to perform duties on weekends.

Required Qualifications:
  • Education sufficient for licensure.
  • National Provider BLS - American Heart Association upon hire.
  • Washington Licensed Practical Nurse upon hire.
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. (Home Health positions).
  • 1 year long term care or related geriatric experience.
  • Experience working with the frail and elderly.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team: Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
