Responsibilites
Provide care to patients under the direction of a registered nurse or physician, functioning within scope of license.
Deliver care to assigned clients and performs direct care in accordance with established plan of care.
Deliver safe care at all times that is in line with physician orders, home health policy and established nursing care standards.
Maintain communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation.
Report patient-related observations to supervising RN on a daily basis.
Administer medications and observe patients for adverse reactions to medications or treatments.
Provide documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirement and assures reimbursement.
Requirements
Licensure / Certification / Registration:
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart
Association or American Red Cross accepted. * Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
Practical/Vocational Nurse credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
Education:
High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
Note: Required professional licensure/certification can be used in lieu of education or experience,
if applicable.
