Summary: The Manager of Community Integration will provide outreach, supervision, coordination, and quality assurance for the Day Habilitation Program. The Manager will ensure that Person Centered Plans/Goals are implemented effectively, appropriate services delivered, and documentation requirements are completed. The Manager will contact area businesses to secure sites for Day Habilitation groups to visit.

Essential Duties and Responsibilities:

  • Interview, hire, train, and supervise Day Habilitation Team Leaders-Groups and Independent Living Coach-Group Leaders.
  • Coordinate daily with Day Hab Leaders to meet program needs and scheduling.
  • Complete annual evaluations of Day Hab Team Leaders and Independent Living Coach
  • Schedule routine supervisions with Day Hab (Group) Team Leaders for clarification and support
  • Coordinate with Day Hab Leaders for quarterly Day Hab team training/building.
  • Plan daily activities to address individual goals focusing on independence, community integration, communication, socialization, leisure activities, self-advocacy, and psychomotor functioning.
  • Attend Case Management meetings to stay abreast of proper services, goals, and any changes to participant Person Centered Plans
  • Perform routine checks of ICM tracker notes and Incident Reports to ensure proper documentation by Independent Living Coaches.
  • Ensure DH group leaders are providing services that meet participants\' individual needs and goals as outlined in Person-Centered Plans.
  • Provide training, support, modeling, supervision, and team building to group leaders to develop their ability to carry out daily services to their assigned participants consistently, effectively, and safely.
  • Assist with developing and coordinating training for all Day Habilitation Group Leaders, ensuring that all COMAR training requirements are met.
  • Develop, establish, and maintain community partnerships that will increase the variety and value of community-based programming.
  • Effectively monitor community-based programming through regular, rotating visits to ensure that all regulatory requirements are being met at all times, including utilizing best practices and participant ratios.
Other Duties and Responsibilities
  • Track and complete the timely evaluations of all CLRS staff supervised.
  • Ensure that group leaders are submitting correct and timely documentation as scheduled.
  • Assist Case Management team with coordinating services and meetings, including group leaders in Person Centered Planning.
  • Provide mobility monitoring and/or training as needed.
  • Coordinate with other League departments to ensure that necessary communication on participant League services occurs and that Day Habilitation works as a team with other services.
  • Attend meetings, conferences, committee meetings, and training as assigned.
  • Implement State of Maryland DHMH, DDA, and League policies.
  • Operationalize the mission of The League for People with Disabilities by creating a supportive environment for participants and staff.
  • Additional responsibilities as assigned by the Director, Meaningful Day, and/or the VP, Community Programs.

  • The ability to work with diverse populations.
  • Organizational solid & managerial skills
  • Excellent written and verbal communication skills
  • Access to a reliable automobile and a valid driver\'s license with no more than 3 points.
  • Mandt, First Aid, and CPR Certification preferred.
Educational and/or Experience:
  • HS diploma required, bachelor\'s degree in human services or related field preferred.
  • Minimum of 5 years experience working with people with developmental disabilities; preference is given to those who have experience with autism spectrum disorders.
  • Minimum of 3 years of supervisory experience
  • Must possess strong computer skills in Microsoft Word Office Suite and skills in internet applications.
  • Must be able to learn an electronic documentation system; preference is given to those who have experience using electronic documentation systems.
  • Knowledge of COMAR regulations and CARF standards related to Day Habilitation services is preferred, but being familiar with regulations within a month of start is a must.
