:This position is a licensed RN, PT, OT or SLP with the skills and abilities to perform home health staff RN or Rehab duties. The Home Health Clinical Manager serves as a communication hub responsible for coordinating the multidisciplinary home health services for the clinical team. This position acts as a liaison between Access Data Entry, Scheduling, Quality and clinical Staff.Providence caregivers are not simply valued - they\'re invaluable. • Bachelor\'s Degree Health care related field.
• within 30 days of hire: National Provider Identifier (NPI) number
• upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
• 3 years In the nursing or rehab field.
