PRIMARY FUNCTIONS

• Performs bio-psychosocial assessments including domains of physical functioning, psychological/emotional functioning, mental status, activities of daily living, instrumental activities of daily living, social functioning, nutrition, financial and environmental These assessments help determine the client\'s needs and are performed in the client\'s home setting.
• Develops service (care) plans based on an assessment to meet the client\'s needs and to establish a strong working relationship.
• Refers for and coordinates needed services internally within MorseLife and externally in the community.
• Acts as a strong advocate to meet clients\' needs.
• Consults and collaborates with other professionals and service providers to ensure comprehensive care.
• Counsels and supports clients\' families and significant others to optimize their well\xc2\xad-being.
• Monitors clients\xe2\x80\x99 ongoing needs through telephone and face-to-face interaction to evaluate the treatment and care plan processes.
• Documents all activity promptly and maintains case records to meet documentation standards.
• Responds to emergencies/crises to maintain quality care.
• Maintains caseload size as required by the agency to meet goals.
• Facilitates support/therapeutic groups to encourage self-help and supportive services for Holocaust survivors and their families.
• Responds to all communications promptly for care continuity.
• Assists with Reparations applications and follow up, as needed.
• Provides anecdotal material to help with PR and fundraising, as requested.
• Provides Person-Centered Trauma-Informed (PCTI} training, as requested.

• Interfaces with other professionals and agencies to coordinate care.
• Attends meetings including staff, department, in-service trainings, CEU programs and other events assigned, to enhance skills, knowledge, and person-centered trauma-informed care and communication.
• Executes on-call functions to assure continuity of care.
• Performs administrative functions such as copying, typing, mailing, and filing to meet documentation and other organizational needs.
• Notifies management for assistance when physical demands or environmental conditions might represent an increased risk to client\xe2\x80\x99s safety.
• Analyzes and discusses billing issues to address fiscal pressures.
• Prepares and enters statistics for outcome measurement.
• Supervises MSW interns, as requested.
• Maintains all practices of HIPPA standards.
• Driving to/from or on behalf of clients.
• Responds to client emergencies or crises on nights and weekends.
• Demonstrates necessary skills essential for this position, including problem-solving, analytical ability, patience, flexibility, and the ability to function independently.

• Licensed Clinical Social Worker or LCSW eligible.
• Masters\xe2\x80\x99 Degree in social work.
• 10+ years working with individuals and families with histories of trauma.
• Knowledge and understanding of and compatibility with Jewish values and traditions.
• Must have a car and a valid Florida Driver\'s License with proof of insurance per agency requirements and the ability to travel within Palm Beach County.
• Demonstrated excellent communication and soft skills and the ability to develop and maintain strong relationships with clients, their families, and service providers.
• Ability to work well within deadlines on multiple and varied tasks simultaneously.
• Strong computer skills and proficiency with Microsoft Office.
