Position Description

The Nurse Practitioner is responsible for managing health problems and coordinating high quality primary medical care, with a focus on women\xe2\x80\x99s and reproductive health, in partnership and collaboration with HONOR COMMUNITY HEALTH\xe2\x80\x99s integrated practice team, in support of HONOR COMMUNITY HEALTH\xe2\x80\x99s mission of delivery comprehensive integrated care to vulnerable populations of Oakland County in accordance with State and Federal rules and regulations and the nursing standards of care. The NP will work collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality health care to residents and the community.

This position reports to the Associate Chief Medical Officer and is located at our Pontiac site. Our standard hours are Monday-Friday, 8:00am-5:00pm.

Responsibilities

  • Provide comprehensive family planning, with direction from the Associate Chief Medical Officer under Title X programming, perform reproductive health screenings, exams and procedures for both male and female patients, provide contraception education and counseling, prescribe hormonal contraceptive methods, including emergency contraception. Perform procedures including IUD and implant insertions and removals, upon training and approval from the Chief Medical Officer.
  • Provide pre and post-natal care, including the evaluation of early pregnancy complications, including but not limited to vaginal bleeding, hypertension, and sexually transmitted infections, etc. Collaborates in coordination with OB/GYN in formulating plans of care.
  • Performs routine gynecologic procedures including colposcopy, endometrial biopsy, and pap smears, etc., upon training and approval from the Chief Medical Officer.
  • Obtain a complete history, perform a physical examination and provide a professional diagnosis and treatment plan as part of evaluation and management of a patient presenting with specific medical problem(s) or concern(s). Evaluate and treat acute and chronic illness, including developing a treatment plan and assisting with coordination of care.
  • Document patient care encounters (i.e. history and physicals, patient and patient-related communication both during and after hours, etc.) in an accurate and timely manner in the patient\xe2\x80\x99s electronic medical records.
  • Make appropriate referrals to both internal and external specialties within the organization and/or to non-organizational specialties.
  • Prescribe medication(s) in accordance with Michigan statute and professional practice guidelines.
  • Complete all charts and review and sign-off on all incoming reports (e.g. lab, imaging) and follow-up in a timely manner.
  • Participate in meetings such as monthly clinician/provider, all-staff, peer review, quality improvement and assurance meetings, grand rounds, Title X workgroups, family conferences, huddles, etc. Participate on the HONOR COMMUNITY HEALTH Quality Committee; assist in development of quality improvement policies and procedures.
  • Recommend changes in all clinical areas, and/or in general areas to improve patient flow, medical records, billing practices, and appointment practices, as well as work collaboratively with entire healthcare team. Responsible for direction and supervision of clinical staff, as assigned by CMO.
  • Participate in quality improvement initiatives and activities as directed by the Chief Medical Officer (CMO). Participate in the implementation of and follow Meaningful Use and Patient Centered Medical Home guidelines.
  • Provide after-hours telephone coverage, as assigned by CMO. Adhere to HONOR COMMUNITY HEALTH\xe2\x80\x99s established productivity standards.
  • Performs other duties as assigned.
What are we looking for?
  • Graduate of accredited School of Nursing. Must be a graduate of an accredited Nurse Practitioner program.
  • Must be board certified at hire and will be expected to maintain appropriate board certification, licensure, and membership in professional organizations.
  • DEA required upon hire.
  • Must maintain BLS Certification through the American Heart Association.
  • Must be eligible to obtain and maintain credentials as a Medicaid and Medicare provider.
  • A minimum of 1-2 years of progressive experience as a Nurse Practitioner.
  • Experience in women\'s health
  • CPT and ICD coding experience.
  • Knowledge of and experience with electronic health records (EHR) systems.
